Become a Member

You can become a member in two ways:

1. Fill out this Google form: Become a registered member

2. E-mail the coordinator: austin.pf.group@gmail.com

Please include your name, email address, and whether you are a patient or caregiver.  

If you would like to share your contact information with other registered members only, then please specify that and whether you would like to share name / age / email address / phone number/ and patient or caregiver status (please share only what you are comfortable with; this can be added or removed later).  

There is no cost for becoming a member.
